RESOLVED Paypal Balance & Lookup addon broken by new date picker

The WHMCS-made Paypal Balance & Lookup addon is broken by the new date picker. It just asks for a start date again, and the icon doesn't fit in the box.

You can find the addon in the Apps & Integrations section, it's not on the marketplace.

I've been able to reproduce this locally, so have opened case ADDON-6115 internally with our development team.
