Issue Number and start date being asked for on credit card form?

Does anyone know why these two additional fields would be showing up when a customer is attempting to add their credit card information? 

• Issue Number:
• Start Date:

I'm setup using Authorize.net CIM and have spoken with Authorize.net, everything is setup up properly on there end.

Any help greatly appreciated!  Thank you everyone!

Hi Troy,

are they required fields or can they be left blank?

I don't use Authorize, but i've seen those fields on other forms and they're hardly ever required.

Hey @socomsystems

Some cards do have Issue Numbers and Start Dates this shouldn't stop a client from entering their card details however and they usually can be left blank
