Module Create Failed

Busy testing site and get the following every time WHMCS goes to create account. Account still gets created in whm but no email is sent with account details.

 

Have included full details from audit log to so hopefully someone can advise what is going wrong. Have checked and can not see were there is an existing user name as this is the only account on my account.
