Check Utilities > Activity Log. Is there any sign that WHMCS is attempting to run module create on the server, or an error message?

Module Create Successful shows that the account was successfully created on the server.

Is the welcome email selected in the product configuration page?

Can you send the email manually using the dropdown on the client's product/services tab?
