Given that it says Failed Admin Login Attempt - IP and not Failed Admin Login Attempt - Username means the username they are typing into the login form is not that of an admin. The fact that no username shows up in the email means that either they are not supplying a username or something is preventing the data in the field from transmitting. Have you changed anything in the template (e.g. if you did perhaps the input has the wrong name)?
