Hi,

This behaviour indicates that an error is occurring when placing an order. Please enable the Setup > General Settings > Other tab > Log Errors and MySQL Debug options.

Then either place a new order yourself, or wait for another affected order to come in. Then review the Utilities > Logs > Activity Log at the time of ordering for any errors.

Don't forget to switch the debugging options off afterwards.
