Package not created after New Client signup?

Not sure if I'm posting this in the right forum & if not I apologize.

 

Just started using WHMCS and have been testing my setup. I noticed that sometimes when ordering & creating a new client account, the client account is created but the package is not setup despite the invoice being paid.

 

Currently I have it set to create the hosting account after payment is received, so if a client pays through Paypal immediately after placing the order, the account should be immediately setup. I'm using a 100% Off promo code for testing purposes mainly b/c I can't send a payment to myself.

 

If I place a 2nd Order from the (now) existing client account using the promo code, the package is setup, emails are sent out, etc. no problem.

 

Is this a bug or do I have something set wrong?

I experienced something similar in CPanel. Customer signs up, account is set up in CPanel, but when I try to create the Module in WHMCS I get the following error:

Module Create Failed - Service ID: 53 - Error: An unknown error occured

 

So I set the client's (reseller) account up in WHM and try to import it: account doesn't show up in Import list;

I downgrade the client to CPanel only, make sure the package name matches the proper settings in WHMCS and again nothing.

 

Only two weeks ago I imported more than hundred accounts that were transferred from another server and everything was smooth. So what's happening?
