It may sound obvious but this error triggers when init.php file is included/required two or more times. You should provide more information. Are you probably requiring/including init.php file in an action hook or in a {php} tag of Smarty?

Hello!

I have the same problem and the lasted WHMCS version installed.

The problem happens when Ireply to tickets, send an email, reset client password, send a message, activate pending orders, etc.

How can I solve this without removing the installed modules?

How to find out which module is including the init.php file?
