Ticket Form (remote)

Probally a common question, though i cant seem to find a direct answer, so in advanced if so i apologise....


However the question is simply "how would you go about handeling the creation of a new ticket for unregistered clients (exclusivly) to one spesific department using the required name email and messege format as well as captacha (from whmcs) within PHP in say a contact us page?"
