Kayako submit ticket fields?

With the Kayako integration, our customers are unable to edit/modify/enter anything in the Full Name/ Email fields of submitting a ticket, and because of this they are unable to submit a ticket because those fields are required.

 

What's wrong with this and how can this be fixed? Any settings I need to be modifying to fix this on Kayako or WHMCS's end?
