Help domainchecker.tpl

hello everyone, after update to latest version WHMCS my domainchecker.tpl page stopped working. Can someone please help me to make corrections on the page? Below is the source code of the page.

take a look in admin -> utilities -> logs -> system activity log. The error is likely to show up in there
