Credit Card Holder Name

I am new to WHMCS 5.1.2 so forgive my ignorance because I can't find this in the docs. If a credit card has a different name than the person on the account, how do you list a different "Name on Card". The credit card details form does not have a place for a name so I just assumed it pulled the name on the account?

 

Would I just setup a different billing contact name? Do I need to include middle initial in the first name field?
