If you add more than 1 additional option it isn't formatted correctly within the CSS. They seem to merge together instead of being formatted left, top with a line seperation. This is a default and unamended template, if you need any more info please advise. Please can you advise of a fix in the mean time? There's no table or CSS formatting, it's just "field label: field input field description" with a line break after that before the next, but you can customise via the templates if you wish.
