Account Creation Failed

I placed a test order, marked it accepted, and I get this error:

 

Automatic Setup Error: Account Creation Failed -

 

YET! it did actually create the account on the server!? But the server doesn't send out a welcome message (either the DA server or the WHMCS side).

 

This is Directadmin, pretty vanilla install on the server side.

 

So I uncheck the "create account" and hit the accept order again, and it accepts it and sends the welcome message (from whmcs).

 

Also when I delete the hosting from whmcs for this test domain, it doesn't remove it from the DA server? Is that normal or can I set this somewhere so it will delete?
