Firstly, what is the invoice status "Collections"? How do you change the invoice status to "Collections"? Secondly, is PayPal a supported module for refunds? Thirdly, what is WHMCS v4.0 like integrating with Kayako? No doubt a patch of some type will need to be released by Kayako... Forth, what's the purpose of security questions?
