Serious problem with affiliate system

I thought I had set up my affiliate system to credit my affiliates a fixed amount ONCE per signup. However, I see that my affiliates have been credited once a month.

These are my settings:

Configuration > General Settings > Affiliates

Enable/Disable: checked

Affiliate Earning Percentage: 0

Affiliate Bonus Deposit: 10.00

Affiliate Payout Amount: 100.00

Affiliate Commission Delay: 180

Configuration > Products/Services > (edit hosting package) > Affiliates

Affiliate Pay Type: Fixed amount

Affiliate Pay Amount: 50.00

Affiliate One Time: checked

and yet, looking at

Clients > Manage Affiliates > (edit affiliate)

I see the following (for example):

Date Referral ID Amount

28/12/2008 8 50.00

22/12/2008 7 R50.00

02/12/2008 9 R50.00

30/11/2008 8 R50.00

24/11/2008 7 R50.00

12/11/2008 9 R50.00

09/11/2008 8 R50.00

02/11/2008 7 R50.00

As you can see, the affiliate is being paid out multiple times for the same signup. What am I missing, or is this a serious bug?

ETA: I'm still running 3.7.2

Also, if using PayPal subscription, check that no extra subscriptions has been created by the customer. If they received an early notice of a bill by email and didn't know about there subscription and paid the bill by setting up another subscription instead of letting it go and having the the intial subscrption automamatically pay it.
