Add order problem (2.5.8)

In 2.5.8, if I try adding an order from the admin section and choose a VPS package, the first page asks for domain and offers the standard "Register, Transfer, Use customers own" options. So I chose "use customers own" and enter a domain - only to be greeted with the following error:

 

The following errors occured:

 

* You must enter a hostname for your server

* You must enter a prefix for both nameservers

* You must enter your desired root password

 

Note, however, these options are not on the page as I assume they are on the next page I cannot get to.

 

Placing the order as a customer from their client area seems to work - although the customers root password is encrypted and not viewable, requireing me to make up a new password for them.
