auto creating After Accepting Pending Order

i have a product whit After Accepting Pending Order account creation !

i had write name of package in the product infromation !

and when i click in the acept in a pendig order i see this error

 

 

Server Command Error

Account Creation Failed: An Unknown Error Occurred

 

 

please help me :?
