Selecting "I am a new customer ... create an account" results in blank page

When placing an order, if a user is a new user and selects "I am a new customer and would like to create an account" when they click continue, they get a blank page.

 

I have no idea how long this has been going on, but it might explain why i have not gotten any new customers lately!!!

 

my site is http://www.quik-site.com. If you click on order now you will be able to reproduce this problem. I am running this in a wrapper but have tried it outside the wrapper as well. (just add /support).

 

am running 3.4.1 on Centos.

Check your orderforms/cart/viewcart.tpl file. Try re-uploading it.
