Custom fields on Order Form conditional (Hide / Show options)

 I need help to integrate into configureproducts.tpl on a seperate order form template to do these conditions for a product group.

On a product, in a group, I have Custom Fields but some are not required depending on the option selected in the 1st dropdown.

Dropdown Option 1 - I have lived at this address for more than 30 days

                    If selected - I would like to transfer my existing number (Tickbox)

                                                If selected - Select your current ISP (Dropdown)

                                                Also field - Enter your current landline number (Text Field)

                   Or - I would like a new phone number (Tickbox)

Dropdown Option 2 - I have moved within the last 30 days . 

                       (No Action)

Dropdown Option 3 - Moving to this address

                  if selected - Please tell us your expected move-in date (Datepicker or text field)
