I would ofercer 3 options at the time of cancellation, ask the subject brighter, offer five options with grades 1-5 to by default give the note to the service you used and finally before the final cancellation step, offer a discount to the client can maintain the service, this system would offer the option yes or no and if he chooses yes, the system would open a ticket requesting that the ADM would add credits to the customer, this case is more complicated to add automatically when you click yes, then the option to open a ticket would be good.
