Order Form Issue

Hello all,

 

I've set up a Shared Hosting product (only one on my site). When I go through to order it the page times out and goes to the 404 on the Wordpress site behind the dir.

WHMCS then become inactive for about 30 seconds and inaccessible.

 

The product is a cPanel item with automatic activation after first bill.

 

 

Upon checking the error logs this the error:

Any ideas?

Upon further investigation this seems to be the error:
