Passing variables through POST with custom forms

I am attempting to create a custom form that contains about a dozen text input fields. When I attempt to process that form using the $_POST array, no variables are passed. I can successfully pass checkboxes and radio buttons, but not text fields.

 

I used a simple foreach loop to test what variables populate the $_POST array and that resulted in an empty array. I assume I need to create the variables and form processing code and put that in the templates php file, but can't find any documentation on this.
