Can anyone do a Customer Order form and change the way products are shown in WHMCS?

Hey there - we're trying to find someone who can do a custom order form and we're not having much luck.

 

Additionally, we're looking for someone who can change the way a product shows up in the Shopping Cart, on the Invoice, and in the client area depending on certain criteria (like the name of the bundle).

 

We'd prefer to work with someone in US or Canada. Any recommendations are welcome.
