Adding a new page in whmcs

Hello,

 

I have created a page to add client details and i want to store details in whmcs. I had referred following link

http://docs.whmcs.com/Creating_Pages to create pages. I have created .tpl and .php file and uploaded them in template and root folder respectively. The .php file consist of page initiate and code to fetch data from form. I am able to connect to database but I am not able to store data to database. The form action link navigates to clientsadd.php page. I have also tried to navigate action link to .php page. But nothing works.

Please help me out.
