Two-factor security help needed!

I bought the extra options the duo security and the random tokens.

 

I discovered that when u have to reinstall you're phone where you downloaded the qr prog on you can't make a other qr code

 

Things I tried:

 

Makeing a new admin after I disabled the duo security ( no qr code )

Disable it and Activated it on first admin no qr code it goes strait to the phone number and push screen.

 

 

Now I have to login as admin using the backup key and make a note every time I login or I have no backup key

 

 

It all started when I renewed my license of whmcs.

First I thought is was my phone butt can't get the qr code screen to show up again.

So very time my site push key to iPhone the app gives a error because there no user in it I need the qr code page to make a new user.
