Client End Credit Card submission form edit

Hi There,

 

I Chose overdue Invoices at top of screen, click on a customers name, click on Login as Client (at bottom of client information), click on invoice number, select paynow.

 

takes me to "sitename.com"/billing/creditcard.php

 

I am using J!WHMCS Integration for this.

 

Is there a way to add a check box after, "Tick this box if you do NOT want us to store your credit card details for recurring billing" a new tick box "I understand the charges incurred.' this needs to also save the user IP address and datetime of the forum submission. Any help or direction much appreciated.
