Display Custom Links Based on Service Type

Hello,

 

I would like to add custom URL's to the Product/Services View Detail page based on the specific service type.

 

For example Site Migration Requests, if the customer product is a Shared Hosting Product it should display the Shared Hosting Migration Request Form instead of Reseller Request Form.

 

I had this working in version 4 however due to a file system crash I lost my custom code, hoping someone can point me in the right direction again.
