WHMCS is not adding clients when order placed

HI,

 

WHMCS is not adding clients when order is placed ,when i was testing with WHMCS by making a test order via client area, after finishing all steps when i proceed to checkout it says -An error occured, when i tried from admin area to add a client - after adding all detail when i click on add a client it gives no error but no client is being added. previously it was adding client even from client area as well.PLease help me,i cant figure out what is the issue.
