Issue Number and start date being asked for on credit card form?

Does anyone know why these two additional fields would be showing up when a customer is attempting to add their credit card information? Issue Number: Start Date: I'm setup using Authorize.net CIM and have spoken with Authorize.net, everything is setup up properly on there end. Any help greatly appreciated! Thank you everyone! -Troy
